Cement-associated peri-implantitis: a retrospective clinical observational study of fixed implant-supported restorations using a methacrylate cement
Abstract

Background: Cement-retained fixed implant-supported restorations involve the risk of excess cement, which can associate peri-implantitis. In connection with routine therapy using a methacrylate cement (Premier Implant Cement, Premier Dental Products Company, Plymouth Meeting, PA, USA) to retain fixed implant-supported restorations, complications, that is, inflammations, were developed in some cases. After removing the suprastructure and the abutment, residual excessive cement was found. For this reason, all implant-supported restorations that had been fixed with this type of methacrylate cement were reevaluated and retreated.
Methods: In a retrospective clinical observational study including 71 patients with 126 implants, the findings made during retreatment were documented. In all cases, the suprastructure and the abutment were removed. For recementation, Temp Bond (Kerr Sybron Dental Specialities, Washington, D.C., USA) was used. If an inflammation had developed, a follow-up appointment was scheduled 3–4 weeks later. Results: In 59.5% of the implants, cement residues were identified. Bleeding on probing was diagnosed at 80% of the implants with excess cement and suppuration at 21.3% of the implants.
After removal of the excess cement and recementation with Temp Bond, a 76.9% reduction in bleeding on probing was found at follow-up. Suppuration was not found around any of the implants at follow-up.
Conclusion: Excess cement left in the implant–mucosal interface caused bleeding on probing in most cases and suppuration in some. The removal of excess cement after cementation should be given high priority. In this retrospective observational study, an unusually high number of implants with excess cement after cementation was found with the methacrylate cement applied in the study.

A longitudinal three-center study of craniofacial morphology at 6 and 12 years of age in patients with complete bilateral cleft lip and palate
Abstract
In this longitudinal study, the craniofacial morphology and evaluated soft tissue profile changes, at 6 and 12 years of age in patients with complete bilateral cleft lip and palate (CBCLP) were compared. Lateral cephalograms from 148 patients with CBCLP, treated consecutively at three European cleft centers, Gothenburg (n (A) = 37), Nijmegen (n (B) = 26), and Oslo (n (C) = 85), were evaluated. Eighteen hard tissue and ten soft tissue landmarks were digitized. Paired t test, Pearson&#39;s correlation coefficients, and multiple regression models were applied for statistical analysis. ANOVA and Tukey-B, as a post hoc test, were used to evaluate the increments and compare centers. Hard and soft tissue data were superimposed using the generalized Procrustes analysis. For Nijmegen, the increments of the variables SNA, ANB, SN-NL, SN-ML, NL-ML, Snss, and Snpg were significantly different than the two other centers (p = 0.041 to <0.001). SNPg increments were significantly different between Nijmegen and Oslo (p = 0.002). The three cleft centers followed different treatment protocols, but the main differences in craniofacial morphology until 12 years of age were the growth pattern and the maxillary and upper incisor variables. Follow-up of these patients until facial growth has ceased, which may elucidate components for improving treatment outcome.

Photographs of dental casts or digital models: rating dental arch relationships in bilateral cleft lip and palate
Abstract
Yardsticks have been developed to measure dental arch relations in cleft lip and palate (CLP) patients as diagnostic proxies for the underlying skeletal relationship. Travelling with plaster casts to compare results between CLP centres is inefficient so the aim of this study was to investigate the reliability of using digital models or photographs of dental casts instead of plaster casts for rating dental arch relationships in children with complete bilateral cleft lip and palate (CBCLP). Dental casts of children with CBCLP (n=20) were included. Plaster casts, digital models and photographs of the plaster casts were available for all the children at 6, 9, and 12 years of age. All three record formats were scored using the bilateral cleft lip and palate (BCLP) yardstick by four observers in random order. No significant differences were found for the BCLP yardstick scores among the three formats. The interobserver weighted kappa scores were between 0.672 and 0.934. Comparison between the formats per observer resulted in weighted kappa scores between 0.692 and 0.885. It is concluded that digital models and photographs of dental casts can be used for rating dental arch relationships in patients with CBCLP. These formats are a reliable alternative for BCLP yardstick assessments on conventional plaster casts.

Tooth agenesis patterns in unilateral cleft lip and palate in humans
ABSTRACT:
To characterize tooth agenesis patterns and their overall prevalence in patients with complete unilateral cleft lip and palate (CUCLP).
RESULTS:
Agenesis of at least one tooth was found in 48.7%, and agenesis outside the cleft was observed in 20.9% of patients. The lateral incisor of the maxillary cleft quadrant was the tooth most frequently missing (39.1%), followed by the maxillary lateral incisor (8.7%), and the mandibular second premolar (7.8%) in the non-cleft quadrants. Thirteen different tooth agenesis patterns were identified. Maxillary and/or maxillary and mandibular second and/or first premolars were involved in all patterns.
CONCLUSION:
A higher prevalence of tooth agenesis is observed in patients with CUCLP, even outside the cleft region, compared with the general population. Thirteen different patterns were observed, of which 6 were unique patterns. Certain teeth were involved in all agenesis patterns. Both the prevalence of orofacial clefting as well as hypodontia is more frequently observed on the left side.

Feste Zähne an einem Tag
Ein Fall aus der Poliklinik der Akademie für Zahnärztliche Fortbildung Karlsruhe
Im vorliegenden Fall wird eine Therapieoption bei frakturierten nicht erhaltungsfähigen Frontzähnen dargestellt. In einem operativen Eingriff erfolgte die  Entfernung der tief frakturierten Zähne 11 und 22, die Sofortimplantation mit Augmentation Regio 11 und 22 sowie die Sofortversorgung mit einem festsitzenden Langzeitprovisorium. Nach vier Monaten wurde der
provisorische Zahnersatz entfernt und beide Implantate durch einen festsitzenden Zahnersatz definitiv versorgt.

Wie erleben Zahnärzt eine gerichtliche Auseinandersetzung mit Patienten? Eine qualitative Untersuchung
Background:
A well-established doctor-patient relationship is essential for dental care. Once there is a legal dispute between the dentist and the patient this doctor-patient relationship has failed. Here we show for the fi rst time how dentists experience these lawsuits. We characterise the dentist’s perspectives and investigate their infl uence on the doctor-patient relationship.
Methods: The experience of dentists who were involved in a legal dispute due to a failed dental treatment is pictured in a pilot study on the basis of narrative and problem-oriented interviews. The narrative part of the interview was analysed with the technique of narration analysis. The problem-oriented part of the interview was evaluated content-analytically. From these data a model describing the failure of a doctor-patient relationship was developed.
Results: Communicated burdens and stress were: expenditure of time, duration of the proceedings, fi nancial burdens, limited support and the feeling of being treated unfairly. Consequences taken from the legal dispute were a more detailed documentation and an improvement in patient information. The confl ict in the doctor-patient relationship is transformed to a diff erent platform when the legal profession is involved. This contradictory constellation leads to a break in the doctor-patient relationship. Advice for other aff ected colleagues is: remain calm, aim at an out of court settlement, seek advice from other colleagues and request legal advice, be
proactive, avoid court litigation and in the end draw conclusions from the dispute. There is a need for a higher quality of legal dental experts. A contact platform provided by the dental profession is required for interactive collegial communication.
Conclusion: Lawsuits in cases of a confl ict between the doctor and the patient are very stressful and need an active coping management but also are a chance for the development of the individual dentist and the profession.

Diversity of patients microflora on orthopaedic and
dental implants

The aim of this study was to compare the diversity of microbial colonization on implant material from different individuals. Eubacterial DNA was extracted, separated and sequenced from orthopaedic metallic implant material, tissues or body fluids, and skin of 4 patients as well as from identical dental cement material from 10 individuals after revision and routine removal. Additionally, the composition of the bacterial population of the dental cement and the oral swab sample from one individual after direct extraction of bacterial DNA was compared to extraction after conventional microbiological enrichment. The latter investigation proved that the commonly used cultivation technique gave different results than direct extraction of DNA, especially as regards the detection of anaerobes. Comparing the bacterial colonization of implant materials from different patients showed significant individual diversity. The common focus on a constricted pathogen spectrum may have to be expanded toward a multispecies population. Moreover, the dependence of the bacterial population on the individual host has to be integrated in discussing implant colonization and infection.

Minimal-invasiver Diastemaschluss im Frontzahnbereich mit direkter Komposittechnik
Ein Fall aus der Poliklinik der Akademie für Zahnärztliche Fortbildung Karlsruhe
Im vorliegenden Beitrag wird der erfolgreiche minimal-invasive Lückenschluss eines Diastema mediale im Oberkiefer dargestellt. Mit Hilfe der direkten Komposittechnik konnte der als störend empfundene Dunkelraum substanzschonend, schnell und elegant geschlossen werden. Ein spezieller Silikonschlüssel, eine adäquate Matrizentechnik und ein unkompliziertes Kompositsystem mit unterschiedlichen Schmelz-, Dentin-, Efekt- und Intensivfarben trugen zum Erfolg der formverändernden Restauration bei.
